Footnotes-EIA Testing:

(A) EIA test required for equine over 12 months of age.

(B) EIA test required for equine less than 12 months of age. For age requirement, contact the state veterinarian's office. Arizona, Florida, Idaho, Illinois, Indiana, Kentucky, Massachusetts, North Carolina, North Dakota, Nevada, Oregon, Pennsylvania, South Carolina, and Wyoming: no pending EIA test allowed.

(C) Suckling foals accompanying EIA-negative dams are exempt. Florida, Georgia, Idaho, Illinois, Indiana, Kentucky, Nevada, North Dakota, Oklahoma, Oregon, Pennsylvania, Wisconsin, and Wyoming: no pending EIA test allowed.

(D) EIA test required within six months for sale or auction.

(E) EIA test required for equine from certain states. For specific states contact state veterinarian.

(F) 12 months if state of origin has a state EIA program.

(G) Test chart must accompany animal. Some states require original copy.

(H) Permit and EIA test are pending when horse is shipped.

(I) Permit and EIA test required for National Rodeo Finals. Nevada: no permit required.

(J) EIA test within 60 days if going to public auction.

(K) Oregon horses exempt.

(L) EIA tests required for equine over six months of age.

Footnotes-Certificate of Veterinary Inspection (CVI):

(i) Pre-approved CVI from state of origin required prior to shipment. Required in Michigan on imported exhibition equine only.

(ii) Permit from the state of destination is required prior to entry. Texas: slaughter horses only.

(iii) U.S. origin CVI, endorsed by a USDA approved veterinarian, valid 30 days from date of inspection.

(iv) Complete description of horse including brands or tattoos.

(v) Approved copy of CVI must be submitted to state veterinarian's office after entry.

(vi) State has requirements regarding vaccinations, testing, or other.
